What is Support Coordination?
Support Coordination is a vital service that ensures individuals with disability receive the required assistance to understand and navigate the NDIS system and utilise their NDIS plans effectively to access pertinent services, enhance their capacity to live more independently, maintain relationships, exercise choice and control, and thrive in the community. At daar, we collaborate with our clients to provide exemplary Support Coordination to meet their unique NDIS plan goals.
We offer three levels of Support Coordination, which are determined by the individual’s NDIS plan. Our team of Support Coordinators have a wealth of experience in this field and possess the necessary skill-set to offer specialised support that is uniquely tailored to each person’s needs.
Our Support Coordination service
Our Support Coordination service utilises a person-centered approach that empowers our clients to make informed decisions and exercise control over their lives. We promote inclusivity and foster a supportive environment for our clients to achieve their full potential and independence.
Level 1
This level of support assists participants to understand aspects of their NDIS plan and build their ability to connect with providers and informal, community or funded supports.
Level 2
This medium-level support helps participants better understand and use their NDIS plan, design and build supports, manage funding, resolve crises, and meet service expectations. It aims to increase capacity for maintaining relationships, managing services, living independently, and being included in the community.
Level 3
This is a higher level of Support Coordination, it is for participants whose situations or support environments are more complex with specific high-level risks. This support helps manage challenges in the participant’s support environment so they can access and connect with appropriate NDIS supports.

Support Coordination is a professional service that assists National Disability Insurance Scheme (NDIS) participants to navigate the complex NDIS system and utilise their plans effectively. Our team provides support and guidance by:
Understanding and analysing your NDIS plan to ensure that you get the most out of your funding and supports
Identifying and connecting you with service providers or community services that best suit your needs and goals
Developing comprehensive service agreements with your chosen providers
Negotiating with providers to address any potential conflicts of interests, ensuring that
your best interests are always protected
Providing ongoing support throughout your NDIS journey, including participant in NDIS plan reviews with stakeholders such as LACs, Planners, and other therapy providers
Continuously monitoring your supports and progress, providing feedback and reports back to the NDIA and other relevant parties
Planning and preparing for your annual NDIS plan reviews to ensure that you remain on track to achieve your goals and targets.

NDIA Planner
Discuss with your NDIA Planner, Local Area Coordinator (LAC), or Early Childhood Early Intervention (ECEI) and discuss what level of Support Coordination you would need included in your NDIS Plan.
Support Coordination
Once Support Coordination is included in your plan, your NDIS Planner will provide you with a referral form. Select daar as your NDIS Support Coordinator by completing your NDIS referral form and emailing it to reception@daar.au or contact us by phone on (02) 9133 2500.
Done
We will contact you and arrange a time to meet and discuss your plan, goals, supports, services, and answer any of your questions. We will also provide you a copy of the Service Agreement to review and agree to. This document details how we will work together.
